Assessment Aperto is knowledgeable about the needs of Guests, based on documentation in a professional assessment. A current assessment is required in order to participate in the program. Assessments Include:
• Social, psychological, and medical history
• Criminal history
• Behavior Issues
• Techniques for behavior support and correction
• List of allergies and specific implications
• List of current prescribed medications
• Verification that Guest will self-administer meds
• Diagnosis and specific implications
• Evaluation of cognitive status
• Social, physical, and emotional strengths/needs;
• Daily living functional abilities
• Preferences and choices regarding daily life
• Preferences for leisure and recreation
• A preliminary service plan.

• Insurance info or medical coupon
• Supply of medications with instructions
• List of all items accompanying Guest
• Shoes and clothing for current weather
• Sports and leisure equipment (optional)
• Petty cash (optional)
• Payment for any services not state-paid
